On the personal data forms (physical or digital form) for admission as a patient, you provide us with general personal data (in particular name and contact details). You also provide information about the doctors treating you and your insurance companies. We can also retrieve some of this personal data (patient master data) directly from your insurance card or your insurance company’s database.
